Things to do at the time of discharge from the hospital?

Here are a few things to consider when you are being discharged from the hospital:

1. Follow your discharge instructions: Your healthcare team will provide you with instructions for your care after you leave the hospital. It is important to follow these instructions closely to ensure a smooth transition and to prevent complications.
2. Make arrangements for transportation: If you are unable to drive yourself home after being discharged, you will need to make arrangements for transportation. This may involve arranging for a ride with a family member or friend, or hiring a car service.
3. Fill any prescriptions: If you have been prescribed medications as part of your treatment plan, it is important to have them filled before you leave the hospital. This will ensure that you have the medications you need to continue your treatment at home.
4. Follow up with your healthcare team: You will likely need to follow up with your healthcare team after being discharged. Make sure to schedule any necessary appointments and to keep them as directed.
5. Make any necessary home modifications: If you have been advised to make any changes to your home to make it safer or easier for you to recover, it is important to do so before you leave the hospital. This may include installing handrails, removing tripping hazards, or arranging for home healthcare services.
6. Seek additional support if needed: If you are feeling overwhelmed or uncertain about your care after being discharged from the hospital, it is important to seek additional support. This may involve enlisting the help of family and friends, or seeking out support groups or other resources.

If you are being discharged and you don’t feel good yet, tell the staff that you don’t feel ready to leave the hospital yet. Sometimes, you may look ok for the doctor or the nurse but you may be feeling miserable inside. 

If you need multiple post discharge follow up appointments soon after the discharge, ask if the hospital has a scheduler to get appointments for you. It is easier to get quick appointments if the hospital calls than you doing it yourself.  You can always change the appointment dates if you can’t make it on a particular date.
